What New Patients Need to Know
We’re happy that you’re considering Dr. Kung and our team at Vitality Dental as your new partners in oral care. Your next step is to make sure you’re prepared for your first visit. On this page, you’ll learn about what you can expect during your initial appointment; you’ll also find information about the forms you need to fill out as well as how insurance will be handled. Please call us right away if you feel like you need additional information!
Your First Dental Visit
During your initial dental visit, we’ll need you to fill out a series of forms that will tell us about any preexisting conditions that we should know about. Once you’ve been called back to our treatment area, we’ll take X-rays of your mouth, perform a careful examination, and check for any and all signs of cavities and other problems that require immediate attention. We’ll create a treatment plan based on our findings and remove any plaque or tartar we’ve found on your teeth and gums.
Online Patient Forms
We know that many patients find waiting for their appointment to be a stressful experience. To shorten the amount of time you spend in our dental office, you can fill out your forms online ahead of time before coming to our dental office. Click the link below in order to get started.
